Athletic Paperwork
Required paperwork for Timberline athletes
All athletes will need to have these things completed and on file at Timberline High School BEFORE the first day of tryouts/practices:

ALL incoming 9th and 11th grade student-athletes competing in athletics

Any 10th or 12th grade student-athlete that DID NOT have a physical completed the prior year.
Physicals must be completed MAY 1st or later of the year they will compete to be valid.
Physical Submission:
A copy of physical exams for athletics will need to be submitted to RankOne. You simply log in to RankOne Website and you can upload your physical there. This should be completed PRIOR to the first day of practices!

All athletes are required to complete online paperwork one time each school year. Once completed they are valid for all sports occurring in that school year.
Beginning May 1, 2026, the Boise School District is launching Rank One, a new, easier way to submit student athletic and required PE physical forms - all in one convenient place.

This new system will replace the previous process and will serve as the primary location for submitting participation paperwork moving forward.

In addition to simplifying physical form submission, Rank One will also provide families with convenient access to team schedules, participation documents, and important athletic communications in one place - making it easier to stay informed throughout the school year.
All paperwork should be completed and physical exams uploaded directly to Rank One.

Hard copies of completed physical exams should NOT be turned into the school!!

Download their student’s current physical- Watch Video

Upload it into Rank One during registration

How to register in Rank One:

Go to the Rank One login page

Select "Add Student"

Enter your student’s "Date of Birth" and "Student ID number"

Select "Start Forms"

Upload your student’s physical and required participation documents

If you have more than one child competing in athletics it is NOT NECESSARY to create multiple accounts.

The Boise School District requires that any school activity that will be traveling and staying overnight must have all participants complete and return this form prior to traveling.

Saint Alphonsus is currently contracted to perform baseline assessments and follow-up care for those athlete's who have suffered a concussion while in season for a sport at the Junior High Schools.

They will be following the BSD Management Plan with only minor differences as outlined in the JH Management Plan Document

C3 Logix is an iPad-based assessment system that saves all results in the cloud. There are 8 testing modules included in this system list to the right. To visit the C3Logix website, click on the ICON.

Sports Medicine Staff will be using the SCAT-6 Concussion Assessment Tool (or something similar) to help gather information about an athlete when assessing for a concussion.
